Custom Quilt Order Information

To place an order for a customized quilt, please email me at shannon@monkeydogquilts.com with the following information:
Your information will not be used for any other purposes than ordering your quilt. After your transaction has been completed, all confidential information will be deleted from my records.

WHAT COLORS/PRINTS? WHAT TYPE OF FABRICS? (100% cotton, batiks, etc…)
SIZE OF YOUR QUILT? (If you don’t know standard sizes, please let me know what the purpose of the quilt will be – snuggle and watch TV?, for your bed?, for just your lap? for a baby? )
ADDITIONAL INFORMATION ABOUT YOUR QUILT THAT YOU WOULD LIKE TO SEE IF I CAN INCORPORATE: ( butterflies, dogs, baseball, etc…)
All of my quilts have the Warm & Natural batting, are made in a smoke-free environment, and are gently washed so to be dog/cat hair free before shipping out. All of my quilts will have a label with my information on it and a place for you to personalize it. I will do free repair work for quilts purchased from me within 3 months time of shipping date. (you pay for shipping & handling).
Shipping:
When possible, I will use the Flat Rate Boxes ($5.00-$10.00), but if I can not fit your quilt in a Flat Rate Box, I will use standard USPS mail. I will mail internationally. I will ship your quilt no later than 2 days after payment.
Pricing & Payment:
I charge $3.oo/hour in labor and my cost for materials. My quilt sizes range 32″-120″ and my prices range $60.00-$270.00. Price is determined by the size & design of the quilt.
I accept payment by PayPal or money order/cashiers check. Please wait for my invoice before payment is made so I can include the correct shipping costs. Payment must be made within 3 days after receipt of invoice.
